Community Mission
Partnership Home manages our local Continuum of Care (CoC) which oversees homeless response for Tarrant and Parker counties. The mission of the Continuum of Care is to culivate and create partnerships to collectively impact effective and efficient community solutions for those experiencing homelessness.
WHAT IS A
CONTINUUM OF CARE
The Continuum of Care (CoC) is the collective networks, institutions, and organizations that provide housing and services to people who are at risk of or are experiencing homelessness.
Federally mandated goals of the CoC include:
- Promote a community-wide commitment to the goal of ending homelessness;
- Provide funding for efforts to quickly re-house individuals and families who are homeless, which minimizes the trauma and dislocation caused by homelessness;
- Promote access to and effective use of mainstream programs;
- Optimize self-sufficiency among individuals and families experiencing homelessness.
The technical name for our local Continuum of Care is TX-601 Fort Worth/Arlington/Tarrant County. The CoC is governed by a board composed of community members invested in the issue of homelessness. Click here to learn more about who is on the CoC Board.
PARTNERSHIP HOME'S ROLE
The Continuum of Care is a community effort managed by Partnership Home. The Continuum of Care Board (link) contracts with Partnership Home to perform three primary duties on their behalf:
- Lead Agency: Partnership Home is designated to carry out the activities of the CoC Board, including fiscal and compliance activities; tasks may include management of the annual HUD application, coordination of other funding opportunities, project and system monitoring, meeting management, and other duties requested by the CoC Board.
- Collaborative Applicant: Partnership Home is designated by the CoC Board as the entity that submits the collaborative funding application to HUD for homeless assistance programs. In 2023, Partnership Home secured nearly $20M in funding for our community.
- HMIS Administrator: All organizations providing services to people experiencing homelessness enter data into the Homeless Management Information System (HMIS). As the HMIS Administrator, Partnership Home owns and manages this data, supporting our community in recording, tracking, and reporting results for the Continuum of Care.
